There are 2 very important diseases in Paediatric Otorhinolaryngology(ENT) Otitis Media with Effusion(OME)the commonest disease of childhood after the common cold and acquired Sub-Glottic Stenosis which cuases significant morbidity particularly to premature babies. Since 1985 these 2 Topics have been my main Research focus. In 1985 I began studying the effects of prolonged Otitis Media on the ultrastructure of the Tympanic membrane in an Animal model and continued this research up until 2004.Two major pieces of work achieved included (1)Studies of the Tympanic Membrane in Persistent Middle Ear Effusion,1986. This was part of my BSc in Anatomy degree. (2)The Effects of prolonged Otitis Media with Effusion on the Tympanic Membrane: An Experimental Study in the Rat,2004. This Thesis was carried out for my MCh (Masters of surgery)which I was awarded in 2004 . In 2009 I started studying Sub-Glottic stenosis and have continued this research to the present day. Key achievements so far, (1)Optimising the Use of Mitomycin C in Subglottic Stenosis 2013 This research was a 3 year project For a PhD student under my Co-supervision and was funded by The National Childrens Research Centre(NCRC) and awarded in 2013. (2)MicroRNAs as Novel Theraputic Targets for the Treatment of Subglottic Stenosis,2016. This was an MSc thesis of which I was a Co-supervisor It was awarded in 2016. (3)Investigation into prevention of fibrotic scar formation as treatment for subglottic stenosis.2019-2022. This is a Post Doctoral research project of which I am a co- Principal investigator .Funding Agency National Children's Research Centre (NCRC) The above research has produced a BSc Degree and an MCh degree for myself. I have supervised an MSc and a PhD student who both achieved their respective degrees . I am currently working as a Co-Principal investigator in a post Doctoral research project. All this research has produced 3 significant publications and promoted me to the International stage in the area of Sub-Glottic Stenosis as evidenced by my many invitations as an International speaker ,moderator and Instructor .

  Summary

  Subglottic stenosis (SGS) is a serious condition where the area beneath the vocal cords becomes narrowed due to a buildup of fibrotic tissue (scarring). It commonly results after intubation in premature babies. The ability to modify this excessive scar/fibrotic tissue is an ideal objective in the management of SGS. Treatment of SGS currently involves surgical techniques, which in turn carry a risk of re-scarring (restenosis). Several treatments have been used in an attempt to minimize scarring with limited success. There is a critical need to develop therapies to treat scarring to improve outcomes for children with SCS. The overall objective of this study is to develop a novel drug treatment for SGS. Previous research has led to the identification of a molecule termed BMP3. Our preliminary research indicts that BMP3 could prevent scarring in the airway. Our objective is to test BMP3 as a drug using laboratory based models. We will investigate if BMP3 is lost in the airway following the intubation. We will also test treatment with BMP3 to prevent scarring. These studies will establish if we can use BMP3 to prevent scar formation and stop SGS. By better understanding the processes that contribute to how preterm babies develop SGS we will be able to develop more effective treatments. The long-term goal is to develop BMP3-based therapy that could prevent scarring. This work has the potential to impact all children that undergo intubation, who are at risk from associated scarring. A therapy that would slow or prevent scarring would significantly benefit child health. Scarring is a contributing factor to many diseases including lung fibrosis and arthritis. BMP3 treatment could potentially improve quality of life and protect against fatalities, with scope far beyond SGS.
