Disability Supporting Documents

As part of your registration with the Trinity College Dublin disAbility Service, we will be asked to provide supporting documentation about your disability, health condition, or specific learning difficulty. 

Supporting documentation helps us understand the impact of your disability and identify the most appropriate supports and reasonable accommodations for your studies. 

You do not need to wait until you have all documentation before applying for support. We encourage students to register as early as possible. 

 

Examples of supporting documents include:

  • A letter or statement from a GP or other healthcare professional confirming your diagnosis and describing the impact of your condition. 
  • Reports from consultants, psychiatrists, psychologists, occupational therapists, speech and language therapists, or other specialists. 
  • A diagnostic assessment report for a Specific Learning Difficulty (SpLD) such as dyslexia, dysgraphia, dyscalculia, or dyspraxia/DCD. 
  • Educational Psychology reports. 
  • Documentation relating to previous disability supports received in school, college, or another educational setting. 
  • An Individual Education Plan (IEP), Special Educational Needs (SEN) documentation, or equivalent educational support plan. 
  • A copy of your Learning Educational Needs Summary (LENS) or disability support plan from another higher education institution. 
  • A copy of a DARE (Disability Access Route to Education) application outcome or supporting documentation submitted as part of a DARE application. 
  • DARE Specific Learning Difficulty (SLD) documentation, including Educational Psychologist reports, literacy attainment testing, Educational Impact Statements, and related supporting evidence submitted as part of a DARE application. 
  • Documentation relating to RACE (Reasonable Accommodations at Certificate Examinations), including evidence of accommodations received during State Examinations. 
  • Confirmation that you are awaiting assessment or diagnosis for a condition such as: ADHD, Autism, Specific Learning Difficulties, Mental Health Conditions, Long-term health conditions.
  • Evidence of previous disability support received through another higher education institution. 
  • Reports from recognised screening tools or assessments where a formal diagnosis is still pending.

This form can be used as supporting evidence when applying for disability supports and may help us assess your support needs more quickly. 

This is not a complete list. You should provide any documentation that helps explain the nature and impact of your disability, health condition, or learning difficulty.

You should still register with the disAbility Service. 

Many students begin the registration process while: 

  • Awaiting assessment or diagnosis. 
  • On a waiting list for specialist services. 
  • Gathering documentation from healthcare professionals. 
  • Transitioning from school or another institution. 

In some circumstances, supports can be considered based on self-declaration, interim evidence, or professional confirmation that an assessment is underway. 

Support is based on your individual needs and circumstances, not solely on a diagnosis. 

 

Additional Documentation 

We may contact you to request additional information where: 

  • Further clarification is needed regarding the impact of your disability. 
  • You are requesting non-standard or specialist accommodations. 
  • Funding applications require specific evidence. 
  • Specialist supports, assistive technology, or equipment are being considered. 

Our staff can advise you if additional documentation is required and support you in obtaining it where appropriate.

We may contact you to request additional information where:

You can submit supporting documents by: 

  • Uploading them during the online registration process. 
  • Uploading them through your student disability profile after registration. 

If your documentation is not in English, please provide: 

  • A copy of the original document. 
  • An English translation of the document. 

Where possible, translations should be completed by a professional translator.

You do not need to have perfect documentation before registering. If a disability, health condition, mental health condition, autism, ADHD, specific learning difficulty, or other condition is affecting your studies, we encourage you to begin the registration process as early as possible.
