Biography

I am a consultant orthopaedic surgeon with a specialist interest in complex trauma and joint replacement. I have completed fellowship training at Addenbrooke's Hospital in Cambridge and currently hold clinical appointments at St James's Hospital, Tallaght University Hospital and the Beacon Hospital. I have extensive research interests that mirror my clinical interests and have published widely in the fields of trauma and joint replacement with over 40 publications in leading journals including the BMJ, NEJM and Cochrane. I have over 800 citations of my research and have won awards for my research in joint replacement. I am currently an NIHR grant holder for a large multi-centre trial (ACEFiT trial) investigating the role of total hip replacement in acetabular fractures. I was the departmental Head of Trauma Research at Addenbrooke's prior to returning to Ireland where I oversaw the running of 15 clinical trials. I also have extensive experience in surgical education and have completed a Masters in Surgical Education at Imperial College London and have an interest in mentorship in medical education.

    AceFIT - Acetabular Fractures in older patients Intervention Trial. A feasibility study comparing three methods of treatment of acetabular fractures in older patients; surgical fixation versus surgical fixation and hip replacement versus non-surgical treatment.
    Summary
    Approximately 2,000 older people break the cup part of their hip (acetabulum) each year in the UK. These fractures are a challenge to treat in older patients as the bone is often fragile and in many pieces. There are 2 main treatment options; bed rest and surgery. Surgery can include a plate and screws to hold the fractured pieces of bone in the correct position until the fracture has healed, or a plate and screws combined with a hip replacement. Patients treated with bed rest or with the first surgical option, a plate and screws, are not permitted to walk on their injured leg until their fracture has healed. Consequently these patients often have a prolonged recovery and may have a poorly functioning hip joint afterwards. Treatment with surgery to replace the hip joint along with a plate and screws (second surgical option) avoids the potential problem of a damaged hip joint in the long term and most positively enables patients to start walking on their injured leg immediately. There are potential side effects associated with all three treatments. Currently we do not know which treatment is best. We propose to undertake a study comparing these three treatments in older patients. Patients who have agreed to participate will be randomly allocated to one of the three treatment groups; 20 patients receiving no surgery, 20 plate and screws only and 20 plate and screws combined with a hip replacement. We will collect information from patients (and their carers) about their health before they receive their allocated treatment and collect more information (including complications, health related questionnaires, costs and x-ray pictures) at 6 weeks, 6 months and 9 months after treatment. This will allow us to assess the feasibility of comparing the treatments at the end of a subsequent more definitive study.
