Biography

Dominic Trépel (PhD) joined Trinity in 2017 as an Assistant Professor of Economics and is jointly appointed as faculty for the Global Brain Health Institute (www.gbhi.org) and Trinity School of Medicine. Trépel is also primary investigator in Trinity College Institute of Neuroscience (TCIN) where he directs Trépel lab (www.healtheconomics.ie). Trépel has been an academic health economics for over 15 years, has published over 100 research articles and much of this research efforts are dedicated to the study of dementia, brain health and ageing. Trépel lab currently includes ten PhDs and Postdoctoral researchers working on grants funded by: Health Research Board (HRB), Irish Research Council (Ireland), Research Collaborative in Quality and Patient Safety (RCQPS), Health and Social Care Northern Ireland (R&D Division), Global Brain Health Institute, Brainlat and National Health and Medical Research Council (NHMRC).

Trépel has previously held Senior Research Fellowships with leading centres in health economics including University of York and the Academic Unit for Health Economics (AUHE) at University of Leeds. His PhD examined the 'Economics of Dementia in Ireland and Europe' was supported by the prestigious Government of Ireland Scholarship (IRCHSS) and a Cochrane Fellowship (HRB), and outputs were presented to members of the Irish Parliament.

Trépel is a member of the International Health Economics Association (iHEA), and the Campbell and Cochrane Economics Methods Group (CCEMG). He was part of the scientific committee for the joint meeting of the Coèllge des Economistes de la Santé (CES) and the Health Economists' Study Group (HESG). He also serves on the Editorial Board for Alzheimer's and Dementia Journal, is an associate editor for Frontiers in Health Services and further peer-reviews research for, amongst others the Health Economics Journal, Social Science in Medicine, National Institute for Health Research (NIHR) Library. Trépel has also reviewed research funding applications for NIHR, Medical Research Council (MRC), GBHI Pilot program, US Alzheimer's Association and the EU Joint Programme " Neurodegenerative Disease Research (JPND). Trépel also serves on the management committee of "Dementia Research Network Ireland".

Through his international doctoral training in health economics (from the Swiss School of Public Health (SSPH+)) and from working in a number of leading research centres, Trepél offers a broad technical skillset which includes: Applied Microeconometrics, Statistical Analysis, Health Service Research, Research Design, Economic Evaluation, Cost-Effectiveness Analysis, Health Technology Assessment, Randomised Control Trials, Decision-Analytic Modelling, Health Financing, Analysis of Health Expenditures, Critical Appraisal of Decision Models, Evidence Based Medicine, Systematic Review, Meta-analysis, Medical Decision Making, Outcomes Research, Reimbursement Mechanisms and Productivity Analysis.

    • Family carers are lauded as the backbone of care provision in Ireland and are an essential complement to formal healthcare. Yet their contribution is not explicitly valued and can commonly be overlooked by decision-makers. Drawing on responses of over 1,400 Irish carers surveyed this year, this Civil Society Partner proposes will estimate of the economic value of unpaid caring activity and, for the first time, will disaggregate the monetary value of carer contribution by social and economic situation of the carer and the condition of the cared-for person. The outputs will be influential many ongoing high-level policy and legislative discussions.

    • Health Economic Partnership between Trinity College Dublin and Public Health Agency Northern Ireland
  • Summary
    • In collaboration between the Public Health Agency Northern Ireland (PHA-NI) and Trinity College Dublin (TCD), a partnership has been established that aims to appraise the current basis for public health policy for older people who become frail. To achieve this aim, the central objectives of Phase 1 are to: (1) Critically appraise current evidence that support PHA-NI frailty policy; (2) Identify evidence gaps in knowledge on appropriate intervention strategies and agree appropriate searches to obtain best available evidence on effective strategies, and; (3) Develop a framework to determine the most efficient design of services across NI. In Phase 2, the best available evidence gathered during Phase 1, is incorporated into a decision analytic model that simulates the relative cost effectiveness of public health interventions for frail older people and ultimately aims to inform future public health policy for Northern Ireland.

    • The Economics of Brain Health: A state-of-the-art decision analytic framework to compare a range of potential alternative investments to prevent, treat or care for dementia
  • Summary
    • By 2030, there will be 65.7 million people with dementia worldwide and associated costs will reach €2 trillion per year. With no clinically meaningful treatment yet available, policymakers remain uncertain on how best to allocate resources to tackle dementia. Future health program allocation decisions will ideally account for evolving needs and opportunities. Potential intervention will span prevention, treatment, and care. The toolkit is expected to expand over time as research discovers new intervention options. Optimal investment portfolios will reflect local epidemiology and resources, as well as up-to-date estimates of intervention costs and effectiveness. The goal of this 3-year project (2022 " 2025) is to provide practical analytic tools to support health intervention policy, programming, and funding decisions related to Brain Health and Dementia.

    • Developing a Regional Capacity in Health Economics for BrainLat: A cross-sectional study to inform economic evaluations on dementia in Chile
  • Summary
    • In Chile, there are currently over 200,000 persons living with dementia (PLWD) and, by 2050, this number is forecasted to exceed half a million. The associated national cost of dementia is $183 Million per annum and, by 2050, this will reach $457 Million. Given the implications of dementia, the Chilean Ministry of Health launched `Plan Nacional de Demencia: 2017-2025" (cost: $12.5 Million) and, to support implementation, this project will enhance dementia-specific Health Economics capacity for Chile (and for the wider BrainLat region). This BrainLat Seed Research Grant aims to estimate stage-specific Quality-Adjusted LifeYears (QALYs), and relevant costs, to allow adaptation of international economic models to inform resource allocation decisions in Chile. By providing proof-of-concept, this initiative also lays foundations to scale up dementia-specific health economic capacity to benefit the wider RedLat region.     • Towards an integrated model of care for older adults transitioning from the Emergency Department to the community
  • Summary
    • Ireland like many other countries has seen a steady increase in life expectancy and has an ageing population. Internationally, health and social care systems are recognising that sustainable healthcare strategies for older adults need to move from acute, episodic care to longitudinal, coordinated and integrated care models. International evidence demonstrates that older adults account for up to 25% of all emergency department (ED) attendances and experience high rates of adverse outcomes following emergency care such as unscheduled ED return, unplanned hospital admission and functional decline. Given the significant incidence of adverse events in this population, there is very little research conducted to explore how integrated care may improve outcomes among those who are discharged from the ED. The overall aim of this programme of research is to develop and examine an evidence-based model of integrated care for older people following ED index visit that is planned around their needs and choices, to support them to live well in their own homes and communities. This aim will be realised through collaboration and meaningful engagement by the research team with service users, advocacy groups, health-decision makers and health-practitioners. The objectives include qualitative and quantitative syntheses of international evidence, analysis of local and national routine health data and meaningful engagement with stakeholders to inform the design, delivery and evaluation of an integrated model of care to improve patient, process, clinical and economic outcomes post ED index visit. This programme of work responds to and resonates with key priorities identified in national ageing, emergency and integrated care strategies relating to the development and delivery of a continuum of high quality care services and supports that are responsive to the changing needs and preferences of older adults. The proposal also aligns with key objectives of the WHO Guidelines on Integrated Care for Older People (2017).

    • SENSE-Cog Care: A feasibility study of hearing and vision support to improve quality of life in care home residents with dementia
  • Summary
    • Of the 55,000 people with dementia in Ireland, nearly 17,000 live in nursing homes (Irish care homes). Over 90% of these residents have significant hearing and vision problems that are frequently undetected or under-corrected. Poor sensory function can worsen quality of life for residents with dementia (RwD) by making communication more difficult and increasing confusion and challenging behaviour. The study aims to ascertain the feasibility of delivery, acceptability and tolerability of a sensory support intervention in Irish nursing homes. To explore whether the sensory support intervention may be effective in improving quality of life and other outcomes for RwD and nursing home staff. To explore what Care as Usual looks like in nursing homes, and lastly, to evaluate cost-effectiveness parameters of the intervention. The findings from this study will inform progression to a definitive trial and contribute to emerging evidence of care home research methodology.

    • The Core Outcome Measures for Improving Care (COM-IC) project aims to identify a suite of outcome measures that are aligned with stakeholder goals; and develop guidelines for implementation that ensure Australia"s efforts to improve the lives of people living with dementia are unequivocally realised. The COM-IC Project will use a participatory co-design strategy to incorporate practical knowledge, experience and preferences of all stakeholders as inputs in each step of the process. The participatory co-design framework will ensure the suite of core outcomes identified and recommended to evaluate dementia interventions and care delivery model are important, relevant, authentic and practical for each stakeholder group. Research objectives Primary Objective: Development of a comprehensive and appropriate suite of core outcome measures accompanied by guidelines (developed according to NHMRC guideline methodology) to monitor and evaluate impacts of care delivery models at individual, organisation and system levels, that are informed by all stakeholders. Secondary Objectives 1. To understand, report and collectively prioritise multiple stakeholders" preferences on core outcome measures to evaluate success of interventions and care delivery models for people living with dementia 2. To map the core outcome sets for dementia into the indicators that are routinely collected in the Australian health and aged care systems To achieve consensus of what to recommend as a suite of core outcome measures for dementia. 3. To determine best practices in measuring, collecting, curating, evaluating, reporting and translating the recommended suite of core outcome measures for dementia. 4. To develop methods to monitor the delivery of Commonwealth and State, Health and Aged Care services at the level of Local Hospital Networks.

    • DRNI links leading researchers across several disciplines in the areas of dementia and ND, it is novel in its multi-disciplinary focus and strategic in its objective to enhance research co-operation and co-ordination across the island of Ireland. By facilitating collaborative consensus and policy based projects and supporting knowledge exchange and public patient events DRNI seeks to improve our understanding of the causes and mechanisms of dementia and neurodegeneration, the effectiveness of different models of care and the policy changes required to improve the quality of life for persons with dementia and neurodegenerative conditions and their caregivers. The inter-disciplinary nature of the network is highly innovative and reflects the emerging international evidence of the role of multi-modal influences on dementia and neurodegeneration. It reflects the network members" belief that the integration of knowledge and practice from diverse research traditions has the potential to radically improve our understanding of dementia and ND, in relation to prevention, care and quality of life. To further network objectives DRNI works collaboratively with the Structured Population and Health Services Research Education (SPHeRE) network which strives to increase cross-disciplinary interaction and collaboration in the area of population health and health services research. Combined the networks serve to act as a conduit to link researchers in the field of dementia and ND with statutory and non-statutory bodies, including, funding agencies, government, patient advocacy, charities, health service organisations, industries, and philanthropic organisations.
