Biography

Following her doctorate from the Department of Physiology Trinity College Dublin, Dr Gabrielle McKee worked in the Department of Physiology Royal College of Surgeons in Ireland, Department of Physiology Trinity College Dublin and the Open University, UK. She joined the School of Nursing and Midwifery, Trinity College Dublin in 1997 and is currently Associate Professor in Biological Sciences.

    • Physical activity in the older adult: The aim of MOVEAGE-ACT project aims to empower older people in Europe to adopt healthy lifestyles which include activity and mobility. It aims to identify and address the motivators and barriers to physical activity and develop possible age-appropriate solutions based on these findings. It will organize and high-level public events to promote active ageing events in a number of sites across France and Ireland. to further empower the older adults it will develop and evaluate brief interventions at these events and an behavioural change web based app to motivate maintenance of physical activity.

    • This study is a follow on from the successful survey by Irish Nurses Cardiac Association (INCA) of the members of CCNAP ( 2015)and their barriers to guideline use and a small study funded by Irish Heart Foundation (2007). In collaboration with the The Irish Nurses Cardiac This study is a small study with the Irish Nurses Cardiac Association (INCA). This follow on study utilised the reminder of the funds.     • Implementing and evaluating Health care interventions and innovations: Cardiac rehabilitation Tallaght Project 5 :Cardiac rehabilitation In PAD
  • Summary
    • Patients with peripheral arterial disease (PAD) (lower limb) have a 6 fold increased risk of mortality from cardiovascular disease (CVD) compared to the general population. Risk factors for PAD are similar to those for CVD. Exercise limitations and claudication pain compound these risk factors and accentuate their impact on CVD. Given the very high CVD risk associated with PAD, patients need interventions to protect their overall arterial tree. The aim of this project was to evaluate a PAD risk assessment and rehabilitation programme. A prospective observational study on one site from 2015 -2019. Team members: N. Fallon, C. Edgeworth, P.   • Summary
    • To evaluative the effectiveness of attendance at a Phase III specialised Heart Failure Cardiac rehabilitation programme (CRHFP) on patient's cardiovascular risk factor profile. Purpose: The European Society of Cardiology (ESC) recommend exercise as one of the core components for patients with chronic heart failure, with reported benefits in skeletal, muscle, respiratory, cardiovascular and physiological responses. Exercise training reduces debilitating symptoms, improves functional capacity, quality of life, and significantly improves survival time, while reducing all cause and HF specific cause of hospital admissions. Methods: This is a retrospective longitudinal study that examined profile factors of patients' pre and post the CRHFP. Patients were recruited through a specialised heart failure service to a 10 week exercise and education program. The following data was examined to ascertain if there was any significant changes in the profile factors following attendance at the programme: Quality of life (Minnesota), levels of anxiety and depression (HADS), functional capacity (6Minute walk test and DASI) and anthropometric measurements (Weight, BMI, WC), and Blood Pressure. Results: Data was collected, pertaining to 100 patients. The mean age was 64.5 years (SD=11.1) with half of patients falling within the 67-83 years age group (range 33-83 years). 80% (n=80) were male. 15% (n=15) of patients did not achieve a compliance rate of ≥60%. The reasons for non-completion included Illness/injury 47% (7), no reason 27% (4), return to work 13% (2), domestic issues 7% (1) and lack of motivation7% (1). There was a significant change in four of the ten examined variables. Post CRHFP, patients showed significant improvements in 6 minute walk score: 426 to 466 meters, p<0.001, systolic blood pressure 122mmHg to 115mmHg p <0.005. The Minnesota Living with Heart Failure score improved: 30.5 to 25 p <0.005. The categories of the anxiety and depression scales were merged dichotomously (Normal/Mild & Moderate/Severe). McNemar analysis revealed a significant decrease in the proportion of participants who scored within the moderate/severe range on the HADS Anxiety scale following the programme (5%) when compared with the proportion prior to the programme (18%; n=78, p<0.01). No significant changes were observed on the HADS Depression score. Conclusions: In-hospital and out of hospital HF care has developed significantly, especially in acute symptom control. More recently, emphasis has been put on the long term control of other risk factors. This study adds to this body of literature indicating that attendance at a hospital based Phase III CR programme that provides supervised tailored exercise, with intensive education and support, is effective in reducing risk factors in patients with the lower grades of heart failure. In addition, this study highlighted the positive effects of exercise training on quality of life and exercise capacity.

    • Implementing and evaluating Health care interventions and innovations: Cardiac rehabilitation Tallaght Project 2: Complexity of Care
  • Summary
    • This study aims to explore problems and outcomes that occurred during well-attended phase-III cardiac rehabilitation programmes. Methods: This was a retrospective, single-site study that extracted data from case notes on all patients who attended phase III over 1 year. Results: The study included 227 patients, 75% of which were males, and an average age of 61 years; 69% had acute coronary syndrome (ACS) and attendance was 81%. Risk factor target levels were achieved in the following proportions: hypertension, 99%; LDL, 75%; anxiety, 97%; depression, 99%; and obesity, 30%. A total of 1246 additional problems (average 5.5 per patient) were addressed. These included chest pain, arrhythmias, hypotension, dizziness and light-headedness. Conclusion: The use of an individualised, holistic approach, and addressing the additional problems that arose during cardiac rehabilitation proved effective in delivering a well-attended programme, with successful patient outcomes. However, it did stretch the team and success may be difficult to sustain.

  • Summary
    • Quality of life (QoL) is a broad concept affected in a complex way by the person's physical health, psychological state, personal beliefs, independence level, social relationships and relationship to significant structures of their environment. Several tools have been developed to assess patient's physical, emotional and social wellbeing in coronary heart disease (CHD). The HeartQoL questionnaire was designed to be a single, reliable and valid health related QoL instrument in these patients, to assess baseline health related QoL, for making between-diagnosis comparisons possible, and to assess the change in QoL after treatment. The aims of this study were to measure the outcomes of the HeartQoL questionnaire in patient's attending cardiac rehabilitation (CR), and to identify if there was correlation with emotional scores on the HeartQoL questionnaire and Hospital anxiety and depression scale (HADS) scores. The HeartQoL consists of fourteen items, ten of which focus on physical wellbeing, and four items focusing on emotional wellbeing, together they provide a global score. The reliability and validity of the HeartQoL was established, when used in EUROASPIRE IV survey. Patients completed the HeartQoL questionnaire at commencement and completion of the CR programme. HADS was routinely completed by each patient at commencement of the CR programme, and on completion if scores were elevated. Paired t-tests were conducted to evaluate the impact of the programme on HeartQoL scores over time. Spearman rho analysis was conducted to identify if there was a relationship between HADS scores and the relevant HeartQoL. SPSS 22 was used for analysis. Results There were 45 patients, mean age 61years, 51% has a MI and 58% had had a PCI. A significant improvement post cardiac rehabilitation was found across all three scales of the HeartQoL (Emotional, Physical, Global) (p<0.05). Examination of the relationship between HADS scores (Anxiety and Depression sub-scale) (post programme) and the emotional sub-scale of the HEART QoL revealed a large negative correlation p<0.05. The HQoL questionnaire is easy for the patients to understand and our study demonstrated an improvement in QoL in patients attending CR

  • Summary
    • Background: Patients with peripheral arterial disease (PAD) (lower limb) have a 6 fold increased risk of mortality from cardiovascular disease (CVD) compared to the general population. Risk factors for PAD are similar to those for CVD. Exercise limitations and claudication pain compound these risk factors and accentuate their impact on CVD. Given the very high CVD risk associated with PAD, patients need interventions to protect their overall arterial tree. Aim: To evaluate a PAD risk assessment and rehabilitation programme. Methods: A prospective observational study on one site from 2015-2017. All de novo identified patients and recently treated PAD, were offered a specially adapted 10 week rehabilitation programme, with exercise twice per week, education, and intervention for uncontrolled CVD risk factors, in line with European Society of Cardiology PAD guideline recommendations. Pre and post data was analysed in the powered sample using SPSS version 21, Mann Whitney or Wilcoxan tests. Results: The profile of the 78 participants was: 77% male, mean age 64±9.3, 68% hypertension, 38% diabetic and 41% smokers. There was significant improvements (p <0.05) in systolic blood pressure (140 pre:129 post) total cholesterol (4.2:3.7), LDL (2: 1.8), initial claudication time (min.sec) (2.05:5.2) and distance (M) (100:290), max claudication time (4.5:13.5) and distance (240:715)(Gardiner-skinner protocol) and Quality of Life (VasQoL)(3.2:2.6). Conclusion: This specific PAD rehabilitation programme, which addressed CVD risk factors is the first of its kind to be established nationally. This program was effective in improving functional capacity and quality of life. Increased time to claudication and endurance was associated with increased self-report improvements in activity and pain. With continued audit of this programme, its merits can be established and will help pioneer this approach on a national basis.

    • Master By research study 2013-2015 (Caroline Finn): Predictors of participation in cardiac rehabilitation in the 21st century.Background: Extensive research has occurred across time investigating the many sociodemographic, clinical and organisational factors that influence both enrolment and attendance in Phase III Cardiac Rehabilitation (CR). There has been multiple changes both in recruitment and within cardiac rehabilitation that may have changed the predominance of these factors. Purpose: To inform future development in cardiac rehabilitation this study aimed to identify the main factors influencing both enrolment and attendance at a CR programme collectively in a multinomial model. Methods: The study design is a retrospective, inferential cohort study. The cohort were patients eligible to attend cardiac rehabilitation. Ethical permission permitted the extraction of data from the patient case notes and hospital databases. Multinomial regression analysis was used to identify predictors of enrolment and attendance at cardiac rehabilitation across three groups: attenders, non-attenders/dropouts and non-enrollers. The predictors included were: age, distance to CR centre, gender, cardiac diagnosis, marital status, employment status and cardiac intervention. Attendance was defined as attendance at 60% or more CR sessions. Results: 1592 patients were eligible to partake in cardiac rehabilitation, 997 (63%) enrolled and in CR., was 756 (47%) completed attendance. Within the eligible patients mean age was 63 years, 76% were males, 38% were employed, 34% had a diagnosis of MI and 62% had only medical intervention. The multinomial model was significant (χ2 (18), 255.6, p <0.001, N =568), explaining between 15 to 17% of the variance in attendance. Non-enrollers were younger, not married, not employed and had no intervention, compared to attenders. Non-attenders were younger, not married and not employed compared to attenders. Conclusion: The identification and awareness that younger age, not employed and not married are important factors that influence enrolment and attendance can be used to modify current practices in CR. These factors, and the influence of having non-intervention in the case of non-attenders, could be addressed by targeting increased awareness, support and education. Targeting these groups during Phase 1 CR by providing additional professional support, encouragement, reinforcing messages with written information about the CR program and identifying family / friend, colleague may assist. Further developing Phase II CR with additional telephone follow-up, including those who initialling did not enrol, to facilitate improved continuity of care and providing a better understanding of CR, may provide further encouragement to attend and complete. Increasing emphasis on the long-term benefits of CR may increase the motivation to attend despite the short term financial burden for fuel cost/ public transport / parking associated with attendance. Providing additional support for these groups during Phase III could also assist.
