Learning from Incidents Research Study

HSE National Quality & Patient Safety, in partnership with the Centre for Innovative Human Systems at Trinity College Dublin, is undertaking a research study to evaluate how effectively learning from incident reviews is translated into service improvement across the health service. The study will examine de-identified incident review reports, alongside surveys and interviews with relevant stakeholders, to better understand how findings from incidents are used to improve patient safety and quality of care.

The study has received ethical approval from both the HSE Research Ethics Committee and the Trinity College Dublin School of Psychology Ethics Committee (Approval number 5304), and a consent declaration from the Health Research Consent Declaration Committee (HRCDC), reference number 24-013-AF1/AMD1.
