Please complete the form below in as much detail as possible. It is essential to complete all mandatory fields which are marked*. Letters of Certification of Qualification will be sent to your home address as held on your graduate record, or can be collected from the Student and Graduate Records Office if requested. Upon receiving your formal request, the Student and Graduate Records Office will prepare a letter of certification for issue normally within 2 working days.
