    • A Longitudinal study of the clinical features, stress biomarkers and brain imaging findings in young adults with depression
  • Summary
    • A key problem in clinical psychiatry is identifying biomarkers for psychiatric disorders. The clinical presentation of depression is very variable, but there are no biomarkers to identify one form from another and thus look for targeted treatments. We are examining the clinical features, biomarkers, and neuroimaging data, with an emphasis on stress systems, in first-episode depression in young adults. We are particularly focusing on biomarkers related to the function of the hypothalamic-pituitary-adrenalaxis (HPAA). This study is being conducted in our local psychiatry service based in Tallaght, South Dublin. The population is demographically stable and the clinical services are well developed allowing for a longitudinal cohort study. Our aims are to get a detailed baseline evaluation of the following measures in young adults at their first episode of depression and look at possible changes in these measures over time: - Clinical features of depression, particularly suicidal ideation, biological features and response to treatment; - HPAA: by examining the cortisol awakening response (CAR) and diurnal pattern of salivary cortisol concentrations; and detailed steroid metabolite profile in the urine; - fMRI, DTI and resting state MRI of limbic and associated brain areas; - blood biomarkers, particularly inflammatory and kyneurenine pathway markers - genetic and epigenetic markers. We will observe how these measures change over time: in the first instance at 1 year. The project is a collaboration between the School of Medicine, TCIN, TCD; and the clinical psychiatry services in south dublin.

    • Depression in pregnant women: effects on the baby's stress systems
  • Summary
    • "Fetal programming" is the process whereby fetal systems, via adaption to intra-uterine conditions, become programmed to respond to the post-uterine environment in a similar way. Animal studies demonstrate that baby/pup hypothalamic-pituitary-adrenal (HPA) axis function is programmed by maternal HPA axis during gestation via transplacental cortisol transmission. Cortisol alters neuronal development and reduces hippocampal glucocorticoid receptor (GR) expression. GRs determine the homeostatic set point of the HPA axis because of their function in terminating the stress response centrally. Maternal gestational stress, in animals, results in abnormal HPA axis homeostasis and responses to stressors, and behaviours similar to those in animal models of depression, in the offspring. There is accumulating evidence in humans that high maternal cortisol levels during pregnancy, associated with psychosocial stress, can have a negative impact on baby HPA axis function and confer developmental disadvantage up to adulthood, and depression thereafter. The point where psychosocial stress has a negative impact on the maternal HPA axis remains undefined. In previous work we established that depression during pregnancy is specifically associated with increased diurnal cortisol secretion during the second trimester and that, in a small sample of babies, those born to depressed women had higher cortisol levels and blunted cortisol stress responses. Our main aim is to confirm, in a large sample, that depression in pregnant women defines a clinical state that is likely to impact on baby HPA axis homeostasis and stress responses at week 8 post-birth. Our second aim is to observe how baby HPA axis function, behaviour and development alter over the first year of life in relation to the maternal HPA axis, taking into account maternal-baby attachment and parenting style. We will also examine the hypothesis that epigenetic processes in the GR gene are involved in the fetal programming of the HPA axis.
