Research Expertise

Description

Dr Fiona Lithander (formerly Leahy) holds a Visiting Academic position at Trinity College Dublin School of Medicine since 2012. Fiona is currently based at the Australian National University (2012-current), Australia's most research intensive University and holds an academic position in the ANU Medical School. Fiona held the position of Lecturer in Human Nutrition at TCD 2006-2012. Fiona graduated with a first class BSc (Hons.) Human Nutrition from the University of Ulster, Coleraine, Northern Ireland in 1997. She completed her PhD at the University of Cambridge, UK, under the supervision of Andrew Prentice and Susan Jebb at the MRC Dunn Nutrition Centre in the area of human energy metabolism and appetite control and was awarded UK Association for the Study of Obesity Student Researcher of the Year in 2001 for her work. Fiona moved to the University of Auckland, New Zealand, as a post-doctoral fellow to work with Sally Poppitt and Garth Cooper where she subsequently took up a Lectureship in Human Nutrition. Here she developed her expertise in randomised controlled trials where she examined the effect of specific nutrients, in particular dietary fatty acids, on risk factors for metabolic diseases in healthy participants and in different patient groups. Other areas of interest include the relationships between obesity and inflammation, metabolic complications in type 2 diabetes mellitus, sports nutrition and the effects of dietary intake on arterial function. Fiona moved to Trinity College Dublin in 2006 and has received funding for her research from the National Children's Research Centre, the Irish Heart Foundation and Science Foundation Ireland. She teaches at undergraduate and postgraduate level and is a Registered Nutritionist (RNutr).

    • Novel anti-inflammatory nutritional intervention to improve the metabolic phenotype in overweight and obese 15-18 year old children - insights into potential genetic susceptibility
  • Summary
    • The emerging model of obesity and diabetes (diabesity) is characterised by sub-acute chronic inflammation and insulin resistance. Mechanistic data shows immune cell infiltration into adipose tissue, which generate cytokines and impede insulin signalling. Whilst childhood obesity is increasing worldwide, the impact of this pro-inflammatory state and its prediction of the metabolic syndrome and diabetes is relatively unknown in early life. This project will investigate if a dietary supplement containing anti-inflammatory nutrients will improve insulin sensitivity in 15-18 year old children. Functional molecular analysis of peripheral blood mononuclear cell-derived macrophages pre- and post-supplementation on adipocyte biology ex vivo will provide mechanistic data in relation to the anti-inflammatory insulin sensitising pathways that are modulated by the dietary supplement. This project will also investigate potential interactions between inflammatory genetic variants, the inflammatory phenotype and risk of metabolic dysregulation.

    • The metabolic responses to exercise in Type 2 Diabetes Mellitus (T2DM) suggest that glucose concentrations, production and utilization are all abnormal during exercise in patients with T2DM compared to non-diabetic individuals partly due to attenuated skeletal muscle insulin sensitivity. Maximal oxygen consumption (VO2 max) has been consistently reported to be reduced in T2DM compared with nondiabetics of the same sex, age and activity levels and endothelial function has also been reported to be abnormal at rest, as well as during exercise, in adults with T2DM compared to nondiabetic people. The current study aims to investigate the effect of the peripheral circulation and endothelial function on exercise tolerance in subjects with T2DM

  • Summary
    • Many changes occur within hours of consumption of a meal and humans are repeatedly exposed to these changes in a day. The extent and kinetics of such changes are modulated by a number of factors including the composition of the meal. Humans spend a considerable amount of any 24 hour period in the post-meal state due to the frequency of eating. It is important therefore to understand these acute changes (i.e. 0-4 hours after a meal is consumed) with respect to the effect that they may have on the risk for developing cardiovascular disease.

    • Cellular mechanisms of insulin resistance and exercise resistance in early onset type 2 diabetes mellitus
  • Summary
    • Early onset type 2 diabetes is becoming more prevalent, driven by the epidemic surge in the incidence of obesity. Prof Nolan and colleagues have recently shown that obese subjects with early onset Type 2 diabetes do not respond metabolically to a three-month aerobic exercise-training regime. Equally obese, but normal glucose tolerant, subjects of similar age did respond with a 20% increase in VO2max. This is an important finding with serious implications both for an understanding of the pathogenesis of early onset Type 2 diabetes and for the design of appropriate and effective treatments. The current study aims to investigate the cellular mechanisms of insulin resistance and exercise resistance in patients with early onset type 2 diabetes mellitus.
