(a) Excessive production of cortisol by abnormal adrenocortical tissue, with suppression of plasma corticotropin and CRH (corticotropin-releasing hormone).

(b) Ectopic over production of corticotropin by malignant tumors, especially small cell carcinomas of the lung.
