Biography
Prof. Paul Fearon graduated in Medicine from University College Dublin, and after 5 years postgraduate training in general medicine, he specialised in psychiatry. He completed his training at the Maudsley Hospital, London and was a consultant general adult psychiatrist there for 7 years. As a senior lecturer at the Institute of Psychiatry in London, he headed the Section of Social Psychiatry and Epidemiology where his research interests included the epidemiology and role of social factors in schizophrenia and bipolar disorder. He returned to Dublin to take up his post in St. Patricks University Hospital and Trinity College Dublin in 2008.

- Quality of Care in an Irish Mental Health Service Context
- Summary
- Quality of Care (QoC) measurement is an important tool in assessing, developing and improving services. This is usually measured using factors that clinicians regard as important indicators of outcome, but until recently less attention has been paid to what service users deem important. However, satisfaction with services is associated with treatment effectiveness, and there is evidence that service user-rated data can be more meaningful that clinician-rated data. Despite this, there has been relatively little work in incorporating the user's view into Quality of Care instrument development. Given the importance that 'A Vision For Change', the framework for the future of Irish mental health care, places on the patient's experience, this is a timely opportunity to address this gap. By building on previous work in South East London, our study has two aims: 1) To develop a brief and user-friendly instrument to measure QoC for use in an Irish mental health service setting. 2) To use this instrument to assess the relationship between QoC and outcome (clinical & social) between two and three years after first presentation to our service in 75 people with bipolar disorder and 75 with schizophrenia. In achieving these aims, we will conduct our study in two stages: Stage 1: We will perform a thorough literature review of high quality studies relating to indicators of outcome for both disorders. We will also conduct a series of focus groups of people with these two disorders who attend our services in order to identify what components they consider most important in good QoC. These will be combined into a preliminary QoC instrument which we will then refine and thoroughly field test in order to establish its validity and user-friendliness. Stage 2: We will identify, trace and recruit 150 people (75 bipolar; 75 schizophrenia) who attended our services 2-3 years previously and interview them using the new instrument. In addition we will collect data using well-validated instruments on satisfaction with services, engagement with services, quality of life, global assessment of functioning, course of illness and level of disability. In doing this, we will address three hypotheses: 1) there will be few differences in patient-rated items for Quality of Care between bipolar disorder and schizophrenia; 2) outcome will be positively correlated with quality of care in both groups; 3) patient-rated items for quality of care will best predict social and functional outcome, whereas clinically rated items will best predict symptomatic and clinical course outcome in both groups. We will employ 2 full time research assistants for 3 years and 1 part time user researcher for 1.5 years, working under the close supervision of the applicants. Service user and stakeholder involvement will occur during the entire period of the study. In doing this, we hope to produce an instrument which can, and will, be used to measure QoC in Irish mental health care settings, and which incorporates service users' priorities.

- AESOP-10: A Study Of The Causes Of Ethnic Differences In The Long-Term Course And Outcome Of Psychosis'
- Summary
The MRC-funded AESOP (Aetiology and Ethnicity of Schizophrenia and Other Psychoses) study is a large first-presentation study of schizophrenia and other psychotic disorders. Over 2 years, this 3 centre study identified all people presenting to services with psychotic symptoms in well-defined catchment areas in South London, Nottingham and Bristol. The overall aims of the study are threefold: firstly, to elucidate the overall rates of psychotic disorder in the 3 centres; secondly, to confirm and extend previous findings of raised rates of psychosis in certain migrant groups in the UK; and finally, to explore in detail the biological and social risk factors in these populations and their possible interactions.
Detailed analysis of the data from this study (incorporating nearly 600 individuals with first-presentation psychosis and matched community controls) is continuing, and to date, over 25 publications have resulted from this study. In 2007, the MRC granted funding to follow up this cohort at 10 year follow up, between 2007 and 2011.

- This study, funded by a grant from Guy's & St. Thomas' Charitable Foundation a) explored the relationship between quality of care, ethnicity and clinical and social outcomes eight years after first contact in a cohort of 330 local patients with a psychotic illness, and b) was used to develop a brief, culturally sensitive and user-friendly measure of quality of care, which can be used by clinicians and which focuses on key aspects of care that are important both to service users and in determining outcome.

- Reasons for very long in-patient admissions in acute adult mental health services
- Summary
- The problem of "delayed discharges" has been identified as a major issue within the NHS, especially for acute and community mental health services. Supported by funding from a SL&M R&D grant, we are studying this issue systematically and quantitatively using, for the first time, a case-control design. The project covers all the boroughs served by The South London & Maudsley NHS Trust to include acute (community) mental health services.
A full dataset is being compiled on a complete sample of delayed discharge patients, operationally defined, with a matched control group. We are testing the hypotheses that the main determinants of delayed discharge will be (a) dual and multiple diagnosis (co-morbidity); and (b) lack of provision for residential long-term rehabilitation. Although the study will uncover local factors I anticipate that the findings will be generalisable to most urban settings.
