Form to add a person to an existing licence

AN EFFORT TO STREAMLINE THE LICENCE PROCESS, WE HAVE CREATED A NEW LICENCE
APPLICATION TEMPLATE.
By creating this format we can avoid the re-typing of your application form!
The
form is in seven parts plus the necessary certificates applicable to your
application. Open each
page by double clicking on the hyperlink (page number in blue);
the form will open in ‘Word’. Save
each page on your computer.

Guidelines to fill in licence
SI 566/2002
Please type in the text boxes,
if you finish one line of the box move to the next box rather than pressing
return, if all your text does not fit what I suggest is to make your font text
to a smaller size, in that way you will be able to type more text into each box.
If what you want to enter still does not all fit, you need to edit your
text a little more.
Now you
have your licence application filled in and saved on your computer.
This
will be handy for you in the future if you wish to re-apply for a licence; as
all it will take is some simple editing on your previous application, which will
be already saved on your computer.
Send your application by attachment to bioresources@tcd.ie where it will be
transferred onto The Department Of Health licence application form.
